As a trauma specialist with over 20 years of experience, Lesley Goth, PsyD, has witnessed firsthand the profound impact of sexual assault on individuals, both through her own experiences and those of her clients, regardless of gender. She says, “This has underscored for me the critical need to address this sensitive topic. Shame and self-blame often emerge as common responses to sexual assault, significantly hindering the healing and recovery process.” In her book, Unbroken: A Survivor’s Journey After Sexual Assault, she explores the intricate dynamics of shame and self-blame and their detrimental effects on one's ability to heal. These issues can ripple through various aspects of life, adversely affecting relationships, careers, happiness, and overall mental well-being.


The statistics surrounding sexual assault are alarming. According to RAINN.org and their extensive research, every 68 seconds, an American is sexually assaulted. In addition, 1 out of every 6 American women has been the victim of an attempted or completed rape in her lifetime. About 3% of American men or 1 in 33 have experienced an attempted or completed rape in their lifetime. The aftermath of sexual assault can lead to severe emotional and psychological distress, including anxiety, depression, post-traumatic stress disorder (PTSD), addictions, and other mental health issues. Understanding the gravity of these statistics is essential in fostering a compassionate society that recognizes the need for comprehensive mental health support for survivors.

Survivors may also grapple with feelings of shame, guilt, and self-blame, which can further exacerbate their mental health struggles. Goth shares that numerous clients have recounted that when they finally opened up about their sexual assault, they were often met with skepticism, shame, or blame, leaving them feeling alone and unsupported. She adds that this pervasive stigma can discourage individuals from seeking help or sharing their stories, perpetuating a cycle of silence and suffering. “It is crucial to confront this stigma by cultivating a culture of understanding and compassion, where survivors feel safe to express their struggles without fear of judgment or reprisal”.

 

The role of trauma in mental health


Trauma plays a critical role in shaping an individual’s mental health following sexual assault. The brain's response to trauma can alter how one perceives threats and manages stress, often leading to heightened anxiety and emotional dysregulation. Additionally, trauma can disrupt a person's sense of safety and trust, affecting relationships and overall well-being. Understanding the impact of trauma on mental health is vital for both survivors and those who support them.

 

Goth shared, “After experiencing sexual assault at the hands of a close family friend during my early life, I grappled with body image issues and developed a sense of self-worth that was solely tied to my appearance. This unhealthy perspective led to promiscuity, disordered eating, and struggles with substance abuse. I share my story to illustrate how trauma can profoundly affect one’s sense of identity, often driving individuals to cope in harmful ways to manage the shame, insecurities, and painful memories associated with their experiences.”

 

Effective strategies for healing and recovery


Healing from the trauma of sexual assault is a deeply personal journey, often requiring a multifaceted approach to address the complex mental health challenges that arise. Goth says, “Healing is not a linear process and needs to be approached with patience and compassion”.
