What happens when metabolic health, hormonal transitions, and skin biology intersect? The term “Ozempic face” has become increasingly common in aesthetics. It is often used to describe facial hollowing, increased laxity, and changes in facial contour observed after significant weight loss associated with GLP-1 medications.

But the biology is more complex than the name suggests. GLP-1 receptor agonists do not simply “age” the face. Rather, substantial weight loss can reduce subcutaneous adipose tissue and alter the structural relationship between facial fat compartments, skin, and underlying support.
At the same time, many patients may experience these changes during another major biological transition: perimenopause and menopause. This creates an important clinical intersection. Weight loss can change facial volume, while hormonal transition can alter the biological environment that supports skin quality, collagen, hydration, elasticity, and tissue resilience.
When weight loss meets perimenopause
Estrogen plays an important role in maintaining dermal structure, hydration, collagen, and other components of skin integrity. As estrogen declines during the perimenopause process, changes in collagen, elasticity, moisture, and skin thickness can become increasingly apparent.
So, consider what happens when these two biological events overlap. A woman enters perimenopause, her estrogen environment changes, her body composition begins to shift, she starts a GLP-1 medication, she loses significant weight and, suddenly, she looks in the mirror and feels that her face has changed dramatically. That experience is so much more sophisticated than “Ozempic face.”
A longevity approach to skin health
In aesthetics, this is why, before starting treatment or skincare, it’s important to determine what your real skin concern is, such as volume loss, skin laxity, dehydration, texture, pigmentation, etc.
Step one: Assess
Objective imaging and professional skin analysis can help establish a baseline and monitor changes over time. In-clinic technologies can support a more structured assessment and analysis of skin characteristics and visible changes, including any medical history that, as mentioned, may relate to skin health.
Step two: Protect
This includes appropriate cleansing, barrier support, hydration, antioxidants, and consistent broad-spectrum photoprotection. The goal is to reduce unnecessary environmental stress while supporting the skin’s natural functions.
Also, integrating evidence-based topical actives, such as peptides supported by clinical evidence, can help address skin laxity.
The objective is to minimize environmental stress while supporting the skin’s ability to function healthily, regenerate, and repair itself.
Step three: Stimulate
When the primary concern is skin quality, texture, or mild to moderate laxity, evidence-based treatments designed to stimulate dermal and tissue remodeling may help increase collagen production, reduce laxity and pigmentation, and improve overall skin quality. These include microneedling, radiofrequency, ultrasound, laser-based treatments, and many more.
Step four: Preserve
The objective is to create a sustainable treatment plan and skincare routine that supports skin quality, tissue integrity, facial harmony, and healthy aging.

It’s about understanding how the patient’s biology is changing and adapting our approach accordingly to achieve the best results, no matter their needs.
The skin cannot be separated from the biology of the person attached to it. Metabolic health, hormonal changes, nutrition, muscle preservation, inflammation, collagen, facial volume, and environmental exposure are not isolated variables. They interact continuously, but we can still support healthy skin aging.
